Integrating Osteoporosis Toolkit in Your EMR

In collaboration with Osteoporosis Canada (OC) and the eHealth Centre of Excellence (eCE), a comprehensive tool has been developed to assist doctors in adhering to best practices for documenting osteoporosis care. The tool includes a custom form along with links to additional guides and resources, all in alignment with the 2024 Osteoporosis guidelines.

To get started, open the User’s Guide for step-by-step guidance on integrating and using the tool in your EMR system.

Osteoporosis Custom Form in EMR

• Decision Support: Integrated fracture risk, and falls assessments in alignment with 2023 guidelines.

• Efficiency: Guided steps and checklists.

• Patient Identification: Easily identify patients over 65 needing screening.

• Risk Calculation: Calculate fracture risk using FRAX.

• Falls & Mobility: Automated falls and mobility assessments.

• Lifestyle Monitoring: Track calcium intake, weight loss, and vertebral fractures.

Addressing the Care Gap and Reducing Costs

Osteoporosis leads to over 200,000 fractures annually in Canada, yet only 20% of patients receive the care needed to prevent future fractures—this care gap results in costly repeat fractures and increased healthcare expenses.

Half of hip fracture patients had a prior fragility fracture.

Effective medications reducing repeat fractures by 50% cost less than $400/year.

The custom form addresses this gap by:

  • Preventing Fractures: Reducing costly repeat fractures, particularly hip fractures, which average over $20,000 in acute care.
  • Lowering Costs: Proactive care reduces hospitalizations and long-term care needs.
  • Closing the care gap with this tool enhances patient outcomes and delivers substantial healthcare cost savings.
