Over 200,000 fractures happen in Canada each year, yet only about 20 percent of patients receive the care needed to prevent the next one. Half of all hip fracture patients had already broken a bone before.

Ideal for family physicians, nurse practitioners, clinic managers and EMR administrators.

What the toolkit does

It runs inside your EMR rather than alongside it, so bone health assessment happens in the visit you are already having.

Finds the patients

Case finding queries surface patients 65 and over, and anyone with a prior fragility fracture, without a manual chart review.

Scores the risk

FRAX returns 10 year major osteoporotic and hip fracture risk, with femoral neck, total hip and lumbar spine T-scores.

Guides the decision

Risk categorization and prompts follow the 2023 guideline, including repeat BMD timing and treatment thresholds.

Developed in partnership with Amplify Care, Version 3.0 aligns with the Clinical Practice Guideline for Management of Osteoporosis and Fracture Prevention in Canada: 2023 Update and introduces a major upgrade to streamline fracture risk assessment within the EMR. Now available for TELUS Health PS Suite.

Built-in FRAX® calculation

FRAX® now runs directly within the Toolkit, so there is no need to leave the EMR or re-enter patient data. Risk scores are calculated at the point of care, keeping you in your clinical workflow.

Updated clinical guidance

The Toolkit reflects recommendations from the 2023 Canadian osteoporosis guideline and supports evidence-informed fracture risk assessment and management.

Seamless upgrade

Version 3.0 is fully compatible with earlier versions. Patient information already entered carries forward automatically, with no disruption and no re-entry.

Systemic Case Finding and Risk Evaluation, EMR Enabled Osteoporosis Care, is a three year initiative supporting primary care organizations to identify and manage patients at risk of fragility fracture.

Participating organizations use the toolkit as part of a structured case finding approach, moving patients through assessment, risk evaluation and treatment.

women over 50 will have an osteoporotic fracture, and 1 in 5 men.

average acute care cost of one hip fracture, before rehabilitation.

A fragility fracture is a sentinel event. A patient who breaks a wrist at 68 is telling you something about the hip fracture they may have at 78. The gap between that first fracture and any bone health assessment is where most osteoporosis care is lost.

Primary care is the ideal setting to close that gap at scale, which is why the toolkit lives in the EMR rather than in a separate portal.
